Hunter instructions for requesting materials (additional diary forms or envelopes) for Migratory Bird Harvest Surveys.
Notice - Updated on August 29, 2025 - Due to unforeseen circumstances, we are currently experiencing a low inventory of waterfowl envelopes and are awaiting delivery of additional envelopes. As such, there will be a delay in fulfilling waterfowl envelope requests. If you are a past participant and have remaining envelopes from past seasons, please use those envelopes until we are able to ship your new order. If you do not have any envelopes, please keep your tails or wings until your envelopes arrive making sure to record the date and location of harvest. We apologize for the delay and inconvenience.
Who Needs It?
- Migratory Bird Hunter Survey (Diary Survey) participants who need more space to record all of their hunting trips and need more Diary Survey forms
- Parts Collection 'Wing' Surveyparticipants who need additional wing envelopes
Instructions
Do one of the following:
- Return the postcard from your previous envelope order (if you have it)
- Email [email protected] and leave your name, address, hunter number (if you know it), and the number of envelopes you need
- Call the toll-free phone number 1-800-419-2965 and leave your name, address, hunter number (if you know it), and the number of envelopes you need
